Job summary

The Reserve at Fountaingrove Memory Care in Santa Rosa, CA is seeking a compassionate and dedicated Care Partner to provide high-quality care to the residents in our community.

You will assist with activities of daily living, support personal care tasks, ensure privacy and dignity, follow infection control procedures, and complete state-required trainings upon hire and annually. This role offers competitive wages and opportunities to grow.

Qualifications

• High school diploma or equivalent required.
• Food handler, first aid, caregiver and CPR certifications required.
• Must be able to read, write and speak English.

Responsibilities

• Provide assistance with activities of daily living in a person-centered care setting.
• Assist with bathing, grooming, dressing, ambulation and meals as outlined in care plans.
• Respect residents' privacy, dignity and comfort at all times.
• Follow standard precautions and infection control procedures.
• Complete required trainings upon hire and annually.

Caregiver \"Care Partner\"

Responsibilities of the Care Partner

• Provide assistance with activities of daily living in a manner that supports our person-centered care philosophy.


• Provides personal care services as outlined in the individual services plans such as assistance with bathing, grooming, dressing, ambulation, positioning, assist to meals, feeding, change bed linens, laundry, etc.


• Assist in daily requirements and tasks as requested.


• Ensure residents are provided privacy, respect, and dignity.


• Follows standard precautions and infection control procedures.


• Completes state required training upon hire and annually.

Required Experience

• Possess, at a minimum, a high school diploma or equivalent.


• Be at least 18 years of age.


• Food Handler Certification, First Aid Certification, Caregiver Certification and CPR Certification.


• Basic computer skills.


• Must be able to speak, read, and write in the English language.


• State Criminal Background Check and LIC 508 Criminal Record Statement


• LIC 503 Health Screening Report
