Job summary

North East Medical Services PACE seeks a Home Caregiver to provide personal care, light housekeeping, and supervision to participants in their homes under the guidance of the PACE IDT. You will support individuals and families, documenting care with Epic EMR and coordinating with clinical staff.

You will complete the required PACE training, hold current BLS and First Aid, and communicate effectively with the team.

Qualifications

• Must complete and pass the North East Medical Services PACE training curriculum upon hire.
• Current BLS and First Aid required.
• Ability to communicate effectively.
• Preferred: minimum 1 year of experience as a Personal Care Aide, EMT, CNA, or HHA.
• Knowledge of geriatric population.
• Knowledge of safe and sanitary personal care techniques for elderly population with physical limitations.
• Home Health Aide or CNA certification preferred.

Responsibilities

• Provide personal care and light housekeeping in the participant's home per care plan.
• Provide supervision and emotional support as needed for participants.
• Report observed changes in participant conditions to the Home Care Supervisor and/or other clinical staff.
• Observe and report participant status including vital signs and nutritional intake.
• Proactively communicate health and safety concerns to program staff.
• Demonstrate safe practices using proper body mechanics and universal precautions.
• Interact professionally with participants and families/caregivers.
• Participate in social and recreation activities with participants.
• Utilize Epic appropriately and serve as content/workflow expert for resolving issues.
• Performs other duties as required by manager/supervisor.

Job description

Under the direction and supervision of the Home Care Supervisor, the PACE Home Caregiver provides personal care, light housekeeping, and/or supervision to North East Medical Services PACE participants in the participant's home.

ESSENTIAL JOB FUNCTIONS

• Provides assistance in the participant's home according to the participant's individualized care plan, and in coordination with the PACE Interdisciplinary Team (IDT), the participant, and the participant's family. This may include assistance with:
  • Personal care including tasks such as peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation.
  • Housekeeping including tasks such as changing linens, laundry, and light cleaning.
• Provides supervision and emotional support as needed for participants to perform necessary tasks.
• Reports observed changes in participants' conditions to the Home Care Supervisor and/or other PACE clinical staff.
• Observes, tracks and reports participant status, including information such as vital signs, . nutritional intake.
• Proactively communicates participant health and safety concerns to the Home Care Supervisor and/or other program staff.
• Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions.
• Interacts in friendly, courteous, and professional manner with participants and their families/other caregivers.
• Participates in social and recreation activities with participants as appropriate.
• Utilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
• Performs other job duties as required by manager/supervisor.

Qualifications

Required

• Must complete and pass the North East Medical Services PACE training curriculum upon hire.
• Current BLS and First Aid required
• Ability to communicate effectively.

Preferred

• Minimum 1 years of experience as a Personal Care Aide or EMT, or certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA).
• Knowledge of the geriatric population.
• Knowledge of safe and sanitary personal care techniques for elderly population with physical with physical limitations.
• Knowledge of proper body mechanics during provisions of assistance with Activities of Daily Living.
• Home Health Aid certification
• Certified Nursing Assistant (CNA) certification
• 1-year experience working with the frail or elderly population.

LANGUAGE

• Must be able to fluently speak, read and write English.
• Fluency in Chinese (Cantonese) is required.
• Fluency in other languages is an asset.

STATUS

• This is an FLSA Non-exempt position.
• This is not an OSHA high-risk position.
• On-Call and Full-Time available.
