Job summary

Boone Ridge Senior Living in Salem, Oregon, is seeking a compassionate Caregiver to provide hands-on resident care with dignity and respect. You will assist with bathing, grooming, dressing, toileting, meals, mobility, and monitoring comfort, while promptly reporting changes to the Caregiver Coordinator.

Qualifications

• High school diploma or equivalent is required.
• Prior caregiving experience preferred; healthcare or long-term care experience valued.
• Ability to read, write, and communicate effectively in English.
• Must demonstrate patience, empathy, and a desire to support seniors.

Responsibilities

• Provide hands-on care including bathing, grooming, dressing, toileting, and mobility.
• Assist residents with meals, hydration, and feeding as needed.
• Support ambulation and positioning; monitor comfort and address needs promptly.
• Monitor vital signs, weight, and fluid intake/output; document accurately.
• Report changes to the Caregiver Coordinator or Executive Director.
• Document all care and observations in compliance with Mosaic policies.

Key Responsibilities

• Resident Care & Support
• Provide compassionate, hands-on care to residents, including assistance with bathing, grooming, dressing, toileting, and mobility.
• Assist residents with meals, snacks, hydration, and feeding as needed.
• Support residents with ambulation, positioning, and range-of-motion activities.
• Monitor residents' comfort levels and promptly address needs and concerns.
• Health Monitoring & Documentation
• Monitor and record vital signs, weight, and fluid intake/output accurately.
• Report any changes in residents' physical, emotional, or cognitive status to the Caregiver Coordinator or Executive Director.
• Document all care, observations, and resident interactions in compliance with Mosaic policies and regulatory requirements.

Qualifications & Skills

• High school diploma or equivalent required.
• Prior caregiving experience preferred; healthcare or long-term care experience strongly valued.
• Ability to read, write, and communicate effectively in English.
• Must demonstrate patience, empathy, and a genuine desire to support seniors.
• Knowledge of assisted living, memory care, and related regulations preferred.
• Ability to work collaboratively in a fast-paced, team-oriented environment.
