What You Will Do:

You will have the responsibility to help implement customized care plans that encompass solutions to nurture the whole person in mind, body, and spirit. You will treat your clients like family, going above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care to clients in their homes
• Help execute comprehensive personalized weekly schedules and care plans
• Complete daily tasks safely and professionally (ADLs)
• Provide personal companionship, actively engaging with clients to offer social & emotional support
• Assist with personal care activities including hygiene, mobility, incontinence care, light housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment at all times
• Provide transportation to appointments as needed
• Document task completion and report changes in clients' physical or mental condition
