Job summary

Charis Hearts, Inc. is seeking a dedicated caregiver to provide non-medical in-home and community care based on each Client Service Plan.

You will perform client wellness checks, assist with admission documents, and help train other caregivers on Activities of Daily Living in the client’s home. Responsibilities include following all policies, maintaining confidentiality, and providing support with transportation and daily activities.

Qualifications

• Must be at least 18 years old.
• Must have valid State ID/ Drivers License.
• Strong communication skills – both oral and written.
• Reliable transportation to and from client locations.
• Willingness to attend training sessions provided by referring agencies and/or the Tennessee MHDD.

Responsibilities

• Provide client wellness checks and non-medical support.
• Assist clients with admission documents as needed.
• Help train other caregivers on ADLs in the client’s home.
• Follow the client service plan and maintain confidentiality.
• Submit required documentation accurately and on time.

Caregiver Responsibilities

Caregivers at Charis Hearts provide non-medical in-home and/or community care based on the Client Service Plan. Responsibilities may include:

• Client Support & Care
  Perform client wellness checks to ensure their well-being.
  • Assist clients with completing admission documents as needed.
  • Help train other caregivers on Activities of Daily Living (ADLs) in the client's home.
  • Provide non-medical support to agency service recipients.
• Compliance & Documentation
  Be familiar with and follow all rules, policies, and procedures related to service delivery.
  • Submit all required documentation accurately and on time.
  • Adhere to the individual service plan for each recipient.
  • Maintain strict confidentiality of all client records and information unless authorized by written consent.
• Safety & Reporting
  Immediately report any incidents of injury involving the service recipient or yourself to your supervisor.
  • Report any suspected abuse or neglect to the appropriate authorities and your supervisor.
