Job summary

Charis Hearts, Inc. in Tennessee seeks caregivers to provide non-medical in-home and community care under client service plans. You will perform wellness checks, help with admission papers, and train others on ADLs to support client independence.

Qualifications

• Must be at least 18 years old.
• Must have valid State ID/Driver's License.
• Strong oral and written communication skills.
• Reliable transportation to client locations.
• Compassionate, caring nature with a desire to help others.
• Willingness to attend training sessions provided by agencies and MHDD.

Responsibilities

• Caregivers provide non-medical in-home and/or community care based on the Client Service Plan.
• Perform client wellness checks to ensure their well-being.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on Activities of Daily Living (ADLs).
• Provide non-medical support to agency service recipients.
• Be familiar with and follow all policies and procedures related to service delivery.
• Submit all required documentation accurately and on time.
• Adhere to the individual service plan for each recipient.
• Maintain confidentiality of client records and information.
• Immediately report any incidents of injury; report suspected abuse or neglect.

Caregiver Responsibilities

Caregivers at Charis Hearts provide non-medical in-home and/or community care based on the Client Service Plan. Responsibilities may include:

• Client Support & Care Perform client wellness checks to ensure their well-being.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on Activities of Daily Living (ADLs) in the client's home.
• Provide non-medical support to agency service recipients.
• Compliance & Documentation Be familiar with and follow all rules, policies, and procedures related to service delivery.
• Submit all required documentation accurately and on time.
• Adhere to the individual service plan for each recipient.
• Maintain strict confidentiality of all client records and information unless authorized by written consent.
• Safety & Reporting Immediately report any incidents of injury involving the service recipient or yourself to your supervisor.
• Report any suspected abuse or neglect to the appropriate authorities and your supervisor.

To be considered for this role, applicants must meet the following qualifications:

• Must be at least 18 years old.
• Must have valid State ID/ Drivers License.

• Reliable transportation to and from client locations.

• Honesty and integrity in all interactions.
• Willingness to attend training sessions provided by referring agencies and/or the Tennessee Department of Mental Health & Developmental Disabilities (MHDD).
