Charis Hearts, Inc. is seeking caring homecare specialists to support clients in the Gallatin area. Join a statewide provider delivering CHOICES services with a focus on quality, compliance, and compassionate client care within clients' homes.

You will perform wellness checks, assist with ADLs, and complete admission documents while following agency policies. Benefits include weekly direct deposit, health benefits, online training, and fast onboarding.

Qualifications

• Must be at least 18 years old.
• Must have valid State ID/ Drivers License.
• Strong communication skills – oral and written.
• Reliable transportation to client locations.

Responsibilities

• Provide client support and care; perform client wellness checks.
• Assist clients with admission documents as needed.
• Train other caregivers on Activities of Daily Living (ADLs) in the home.
• Provide non-medical support to agency service recipients.
• Follow all rules, policies, and procedures related to service delivery; complete required documentation on time.
• Maintain confidentiality of client records and report incidents or abuse as required.

Caregiver Responsibilities

• Client Support & Care
  Perform client wellness checks to ensure their well-being.
  • Assist clients with completing admission documents as needed.
  • Help train other caregivers on Activities of Daily Living (ADLs) in the client's home.
  • Provide non-medical support to agency service recipients.
• Compliance & Documentation
  Be familiar with and follow all rules, policies, and procedures related to service delivery.
  • Submit all required documentation accurately and on time.
  • Adhere to the individual service plan for each recipient.
  • Maintain strict confidentiality of all client records and information unless authorized by written consent.
• Safety & Reporting
  Immediately report any incidents of injury involving the service recipient or yourself to your supervisor.
  • Report any suspected abuse or neglect to the appropriate authorities and your supervisor

• Willingness to attend training sessions provided by referring agencies and/or the Tennessee Department of Mental Health & Developmental Disabilities (MHDD).
