Job summary

12 Oaks Senior Living is seeking a dedicated Caregiver to provide resident services, including personal care, medication reminders, meals, and social activities to support residents’ well-being. You will review resident Service Plans, assist with medications per regulations, respond to emergencies, and help maintain a clean, safe environment while encouraging participation in activities.

• Must be at least 18 years old.
• Must meet in-service training requirements.

• Must be able to regularly lift up to 50 pounds, occasionally more than 50 pounds, and stand/walk for long periods.

• General understanding of the needs of seniors.

Responsibilities

• Provide resident services including personal care, medication reminders, social and recreational activities, meals, and other tasks to support resident well-being.
• Review resident Service Plans to gain familiarity with needs and preferences.
• Review Staff Communication log and resident Service Notes daily for changes in needs/health conditions.
• Provide personal services as assigned and indicated on Service Plans.
• Help residents with medications and/or treatments as indicated and allowed by regulation.
• Respond to calls from the emergency call system promptly.
• Demonstrate knowledge of and follow infection control procedures.
• Maintain a clean, comfortable, and safe environment by providing housekeeping services as needed.
• Assist with individual and group social/recreational activities and encourage participation.
• Monitor changes in needs/preferences/health status and report/document changes.
• Respond to emergencies per training.
• Report any abuse or suspected abuse to the supervisor and/or agencies.
• Perform other duties as assigned.
