Job summary

Sharecare, Inc. through CareLinx seeks caring In-Home Caregivers in Sylmar, CA to provide non-clinical, in-home support in client homes. Responsibilities include ADLs assistance, documentation via app, and proactive communication with the CarePartner team.

You choose flexible part-time shifts based on availability. Qualified candidates have 6+ months caregiving experience, a smartphone with the CareLinx app, and a valid background check.

Qualifications

• Minimum 6 months of caregiving experience or relevant education/certification.
• Accessto a smartphone capable of running the CareLinx app.
• Ability to communicate in English; bilingual encouraged.
• Reliable transportation to and from client homes.
• Must pass a background check.
• Program-specific training may be required before shifts.
• Medical clearance such as TB test or physical exam may be required.
• Valid driver's license and MVR check for driving shifts.

Responsibilities

• Assist clients with activities of daily living (ADLs) including companionship, meal prep, mobility support, bathing, grooming, and non-clinical medication reminders.
• Complete and document care tasks accurately in the CareLinx mobile app during each shift.
• Monitor and report changes in client condition to the CareLinx clinical team.
• Submit timesheets on time through the CareLinx app.
• Communicate with CarePartner about case updates or client changes.
• Maintain patient privacy, dignity, and safety at all times.
• Ensure confidentiality of client information.
• Provide additional support such as transportation, hospital discharge escort, and telehealth navigation when needed.

WHAT YOU'LL DO

Core responsibilities for all caregivers:

Assistclients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping

Complete and document all care tasks accurately in theCareLinxmobile appduringeach shift

Monitor and report changes in a client’s condition or self-care abilities to theCareLinxclinical team

Submit timesheets on time through theCareLinxapp

Communicate with your CarePartnerregardingany case updates or client changes

Ensure patient privacy, dignity, and well-being in a safe and secure environment

Maintain client confidentiality at all times

Additionalshift opportunities (based on client needs):

Transportation and errand support

Hospital discharge escort — safelyaccompanyingmembers home after discharge

Telehealth navigation — helping members connect with virtual doctor visits

WHAT WE'RE LOOKING FOR

Required qualifications:

Minimum 6 months ofcaregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)

Accessto a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets)

Ability to communicate in English (bilingual caregivers strongly encouraged to apply)

Reliable transportation to and from client homes

Must pass a background check

Program-specific requirements (may apply depending on assignment):

Completion of program-specific training prior to accepting shifts

Medical clearance such as TB test, physical exam, or drug screening

Valid driver's license and MVR check if accepting driving shifts

Additionaldocumentation may berequiredbased on state law or client contract
