What You Will Do:

You will implement customized care plans that support the whole person—mind, body, and spirit. Treating clients like family, you will go above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care in clients' homes
• Execute personalized weekly schedules and care plans
• Complete daily tasks safely and professionally (ADLs)
• Provide companionship and social & emotional support
• Assist with personal care activities, mobility, incontinence care, light housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment
• Transport clients to appointments as needed
• Document task completion and report changes in client condition
