Job summary

Sapphire Health Services, LLC in Eugene, Oregon, is seeking a compassionate Caregiver to join our Memory Care and Assisted Living community. You will provide hands-on assistance with daily living activities, ensure resident safety, and foster a positive, engaging atmosphere for residents receiving specialized support.

This role emphasizes teamwork, accurate record-keeping, adherence to care plans, infection control, and respectful communication with residents, families, and staff.

Qualifications

  • Must be 18 years of age.
  • High school diploma or equivalent is required.
  • Experience in caregiving preferred.
  • Ability to work independently and as part of a team.
  • Ability to pass a background check.
  • Good organizational and time management skills.
  • Ability to perform physical tasks and assist residents with mobility.

Responsibilities

  • Provide assistance with activities of daily living (ADLs).
  • Provide companionship, emotional support, and meaningful engagement to residents.
  • Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports.
  • A Caregiver will maintain resident confidentiality.
  • Encourage clients to participate in activities that promote physical, mental, and emotional well-being.
  • Support clients in maintaining independence in their daily routines and decision-making.
  • Ensure a safe, clean environment by identifying and addressing potential hazards in community settings.
  • Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.
  • Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care.
  • Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes.
  • Present for staff meetings and training.
  • Monitor and document changes in residents’ physical, emotional, or cognitive conditions and promptly report concerns to nursing staff or supervisors.
  • Assist residents during mealtimes, including serving meals, encouraging proper nutrition, and providing feeding assistance when needed.
  • Follow individualized care plans and comply with state regulations, company policies, and infection control protocols.
  • Other duties as assigned.

JOB SUMMARY:

  • Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility.
  • Must be able to stand, walk, and reach for extended periods.
  • Ability to pass a background check.
