Job summary

Sapphire Health Services, LLC is seeking a dedicated Caregiver for our Community Based Care Facility in Oregon. You will assist residents with daily activities, promote independence, and ensure safety and well-being, while providing companionship tailored to each individual's needs.

Applicants should be at least 18 with a high school diploma or equivalent; prior caregiving or home care experience is preferred.

Qualifications

  • 18 years of age minimum.
  • High school diploma or equivalent required.
  • Previous caregiving, home care, or community-based care experience preferred.

Responsibilities

  • Provide assistance with activities of daily living (ADLs).
  • Monitor and report any changes in residents' physical or mental health.
  • Assist with serving resident meals.
  • Provide companionship and build positive relationships with clients to reduce feelings of isolation or loneliness.
  • Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports.
  • A Caregiver will maintain resident confidentiality.
  • Encourage clients to participate in activities that promote physical, mental, and emotional well-being.
  • Support clients in maintaining independence in their daily routines and decision-making.
  • Ensure a safe environment by identifying and addressing potential hazards in community settings.
  • Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.
  • Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care.
  • Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes.
  • Present for staff meetings and training.
  • Other duties as assigned.

Requirements


  • Must be 18 years of age

  • High school diploma or equivalent required.

  • Previous experience in caregiving, home care, or community-based care is preferred.

  • Ability to work independently and as part of a team.

  • Patience, empathy, and a genuine desire to help others.

  • Good organizational and time management skills.

  • Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility.

  • Must be able to stand, walk, and reach for extended periods.

  • Ability to pass a background check.

WHO WE ARE: One of our newest communities, The Amber was acquired in November 2022. The community has a license for up to 40 assisted living residents and has 32 total units. Located in the tiny town of Clatskanie many of the residents are long time community members. The community has a newly renovated employee breakroom and family like staff.
