What You Will Do:
Implement customized care plans that promote the well-being of clients in mind, body, and spirit. Treat clients like family, going above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care to clients in their homes
• Assist in executing personalized weekly schedules and care plans
• Perform daily tasks safely and professionally (ADLs)
• Provide companionship and social/emotional support
• Assist with personal care activities including hygiene, mobility, incontinence care, light housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment
• Transport clients to appointments as needed
• Document tasks and report changes in client condition
