Our Home Care Assistants are vital team members, providing aid in participants’ homes and contributing to care planning as part of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the Home Care Assistant will deliver safe personal care services to WelbeHealth participants in their homes, as scheduled and directed.

Essential Job Duties

1. Provide hands-on personal care and assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and physical transfers.
2. Assist with Instrumental Activities of Daily Living (IADLs), including laundry, light housekeeping, shopping, etc.
3. Measure and document participants’ vital signs as instructed.
4. Help with light meal preparation and feeding to ensure participants are nourished and hydrated.
5. Assist with safe transfers and act as medical escort to appointments as needed.
6. Support the Rehabilitation department by executing home exercise programs per care plans after training.
7. Provide translation services if qualified.

Job Requirements

1. High School Diploma or Equivalency.
2. Current BLS/First Aid Certification.
3. Reliable transportation.
4. Bilingual skills preferred.
5. Must be registered and cleared with the Department of Social Services Home Care Registry before starting.

Compensation

Hourly wage range: $21.00 - $25.22 plus bonuses and equity, based on experience.
