Job summary

PurposeCare is seeking caring, dependable individuals to join our home health team, providing one-on-one care in patients' homes and communities. You will perform personal care tasks, support daily living activities, and help maintain safety and independence for clients across shifts.

Training and certification opportunities are available for motivated candidates. We value compassion, professionalism, and reliability, with flexible scheduling across full-time, part-time, or PRN opportunities.

Qualifications

• Must-have items: Age 18 or older, ability to follow written care instructions, compassionate and professional demeanor, reliable transportation and ability to travel to client homes, ability to lift up to 25 pounds, willingness to complete TB testing, background screening, and onboarding requirements.

Responsibilities

• Provide personal care including bathing, grooming, dressing, oral hygiene, and toileting.
• Assist with mobility, transfers, ambulation, and ROM exercises per care plan.
• Prepare meals and assist with feeding as needed.
• Support medication self-administration through reminders (not administering).
• Monitor and report patient condition to supervising nurse.
• Take and record basic vital signs when trained.
• Maintain a clean, safe client environment.
• Assist with linens and light laundry.
• Use safety equipment properly.
• Document care provided according to standards.
• Travel reliably to patient homes and visits within service area.
• Complete required in-service training, including dementia education.

You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Key responsibilities

• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

Must-have requirements

• Age 18 or older
• Willingness to complete competency and skills assessments
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations
• Ability to work independently in home settings
• Reliable transportation and ability to travel to client homes
• Ability to lift up to 25 pounds
• Willingness to complete TB testing, background screening, and onboarding requirements

Nice-to-have

• 6+ months of caregiving or direct patient care experience
• Home health or home care experience
• Experience supporting seniors

Work environment & schedule

• One-on-one care in patient homes and occasional facility settings
• Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
• Assignments may include homes with pets or smokers
• Travel within your preferred coverage area
