Job summary

Placement Partners in Minnesota is seeking an Employment Support Specialist to help individuals with disabilities gain independence through job coaching, career exploration, and life-skills training in community settings.

You will meet clients in the community, at workplaces, and occasionally in their homes to support employment success and long-term retention, collaborating with employers, families, and care teams, while documenting progress and developing individualized goals.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED required.
• Valid driver's license, reliable vehicle, and proof of auto insurance.
• Experience with individuals with developmental disabilities is preferred, not required.

Responsibilities

• Help individuals with disabilities explore career interests and secure competitive community employment through assessments and planning.
• Provide resume creation, employer outreach, application assistance, interview prep, and job offer guidance.
• Deliver on-the-job coaching and mentoring, supporting onboarding, scheduling, transportation and barrier resolution.
• Promote long-term employment retention through ongoing training, advocacy and coordination with employers and teams.
• Facilitate community integration and life skills development through person-centered support and progress tracking.
