Voa Gny in the Bronx, NY is seeking a Case Manager/Economic Empowerment Specialist to support individuals with disabilities through the Career Access Program. You will identify eligible participants, craft individualized career access plans, and connect clients to essential resources and benefits to help them succeed in career and training goals.

Collaborating with community organizations and agencies, you will ensure comprehensive support and guidance, while documenting services in compliance

Qualificações

• Bachelor’s degree or 60 college credits and 3+ years in workforce development or vocational rehabilitation.

Responsabilidades

• Develop individualized service plans and provide referrals, counseling, and support services to achieve goals.
• Conduct brain injury screening and provide psychoeducation, advocacy and support.
• Refer for cognitive assessment, treatment and rehabilitation services.
• Assist participants in accessing benefits and social services, including SSI/SSDI via SOAR.
• Provide financial counseling related to credit repair and budgeting.
• Build relationships with community organizations and government agencies.
• Document all services in line with policies and laws.
