Job summary

Maine Medical Center, the flagship hospital of MaineHealth, seeks a transition coordinator in Medical Affairs to support discharge planning and payer communications across Maine/NH. You will coordinate with the treatment team, patients, families, PCPs, post-acute providers, and insurers to ensure timely and accurate information delivery.

Requirements include an Associate's Degree or three years of medical experience; one year in a healthcare setting preferred.

Qualifications

• 1 year of experience in a healthcare setting preferred.
• Knowledge of discharge dispositions and Medicare/Denials communications is beneficial.
• Ability to coordinate with multiple stakeholders (treatment team, patients, families, PCPs, post-acute providers, insurers).

Responsibilities

• Implement transition plans with the treatment team and families.
• Communicate with patients, families, and providers; manage discharge dispositions.
• Facilitate transportation arrangements and relay payer information to care managers.
• Enter and transmit patient records and discharge documents (e-discharge).
• Collaborate with insurance companies as directed.
