Qualifications

• Bachelor’s degree in Social Work (BSW) required.
• Social Work Certification required.
• 2+ years of experience in a healthcare setting.
• 2+ years of experience with healthcare terminology.
• Ability to read, write, speak, and provide emotional support.

Responsibilities

• Coordinate patient care processes to achieve quality outcomes and optimize resources.
• Provide direct social work/case management services to patients and families.
• Discharge planning and collaboration with the care team.
• Connect patients with community resources and financial assistance.
• Respond to information and referral needs of the community.
• Maintain compliance with CMS and Joint Commission Standards.
• Participate in department quality improvement projects.
• Initiate paperwork for reports, plans of care, referrals, and resources.
• Coordinate discharge plans for all age groups.
