Qualifications

• Bachelor’s degree in social work (BSW) is required.
• Social Worker Certification is required.
• 2+ years of experience in a healthcare setting is required.
• 2+ years of knowledge/experience in healthcare terminology is required.
• Ability to read, write, and speak clearly and support patients emotionally.
• Onsite work at hospital location(s) in Waukesha WI.

Responsibilities

• Coordinates patient care processes to achieve quality outcomes.
• Provides interventions for patient/family support and crisis resolution.
• Assesses need for post-hospital services and community resources at discharge.
• Develops care plans with physicians, ED providers, and community resources.
• Educates patient and family on care options within appropriate level of care.
• Complies with CMS and Joint Commission Standards.
• Discharges planning and coordination across age groups.
