Job summary

UF Health in Gainesville seeks a Pharmacist – Transitions of Care to provide comprehensive medication management across care settings. Qualifications

• Strong communication skills with patients and teams.
• Ability to positively interact with health-system staff.
• Pharmacy licensure in Florida or eligibility.
• Hospital pharmacy experience highly desirable.

Responsibilities

• Perform comprehensive medication reconciliation for patients at admission and discharge.
• Identify medication discrepancies and opportunities to optimize therapy.
• Develop and communicate medication recommendations to providers and care teams.
• Provide patient and caregiver education to promote medication understanding and safe use after discharge.
• Support discharge medication planning, including access and coordination with outpatient pharmacies.
• Facilitate safe transitions between inpatient, outpatient, and post-acute care settings.
• Document clinical interventions and medication reconciliation in the EMR.
• Collaborate with interdisciplinary teams to improve medication-use safety and efficiency.
• Participate in initiatives to reduce medication-related adverse events and readmissions.
