Job summary

Catholic Charities New Hampshire is hiring a Transitions of Care Coordinator for Health Care for the Homeless in Manchester. You will help patients navigate complex healthcare systems and ensure they don’t fall through the cracks during critical moments of care.

You will coordinate specialty referrals, obtain prior authorizations, track referrals, arrange transportation, and support patients as they access medical and social services, working with clinical and quality teams.

Qualifications

• High School Diploma or GED and 3–5 years of relevant healthcare experience.
• Associate’s or Bachelor’s degree preferred.
• Basic computer and data entry proficiency.
• Compassionate, patient-centered approach to working with vulnerable populations.
• Valid driver’s license or reliable transportation for occasional travel between sites.

Responsibilities

• Coordinate specialty referrals, diagnostic testing, and community partner services
• Obtain prior authorizations and communicate outcomes with providers
• Track referrals and follow up on incomplete appointments to prevent gaps in care
• Support smooth transitions following hospital discharge or specialty visits
• Arrange transportation and escorts to promote appointment adherence
• Assist patients in navigating healthcare and social service systems
• Monitor insurance portals and medication formularies and share updates with the team
• Collaborate with clinical and quality staff to identify patients needing outreach
• Support front-end intake and registration processes as needed
