Job summary

San Francisco Campus for Jewish Living (SFCJL) is seeking an Admissions Coordinator to support admissions to the Jewish Home & Rehab Center. You will liaise with hospitals, referrals, and interdisciplinary teams to ensure smooth transitions for new residents and help achieve census and revenue goals.

You will manage data entry across EMR systems, assist with applications, and maintain strong relationships with referral sources while delivering high-quality customer service in a fast-paced

Qualifications

• High school diploma or equivalent; additional degrees preferred.
• Experience with MS Office (Outlook, Word, Excel) required.
• Ability to multi-task with urgency and sensitivity.
• Excellent interpersonal, organizational, and communication skills.
• Experience in fast-paced healthcare environment preferred.

Responsibilities

• Support census and throughput goals in LTC admissions.
• Manage admissions data entry across EMR, Allscripts, Curaspan, Referral Manager PRO, Fax, Phone, Email, and LTC roster.
• Admit short-term to LTC and manage the application process prior to patient arrival from hospital.
• Communicate with Case Managers about admissions to ensure smooth transitions.
• Review Long-Term Care applications for completeness and eligibility; follow up with applicants.
• Coordinate with patient financial counselor for insurance/ Medi-Cal where needed; verify payer information.
• Maintain waitlist and communicate move-ins to LTC; arrange room prep and DME.
• Build relationships with referral sources, physicians, and other healthcare professionals.
• Attend staff meetings and cross-train as needed.
