Job summary

Memorial Home Services is seeking a Patient Discharge Specialist to assist in coordinating patient discharge plans from the hospital to extended care facilities, home, or other levels of care. The role involves verifying insurance benefits, educating patients and families on options, and coordinating with providers and payors to implement the discharge plan.

Responsibilities include coordinating transportation, documenting Medicare/Medicaid coverage, and ensuring timely communication across care

Qualifications

• Two years minimum experience in acute or long-term care.
• Understanding of healthcare reimbursement mechanisms.
• Strong oral and written communication skills required.
• Customer service experience and/or training preferred.
• Office computer skills required.

Responsibilities

• Discharge coordination: work with Patient Care Facilitator/Social Work staff to coordinate discharge plans.
• Discuss discharge preferences with patients and families, elicit facility or home health options, and coordinate referrals.
• Coordinate transportation needs and ensure proper documentation for Medicare/Medicaid coverage of discharge services.
• Communicate with insurance carriers to verify benefits and obtain in-network lists.
• Prepare and disseminate discharge documentation to providers and payors, and obtain signatures on discharge worksheets.
• Participate in ongoing departmental activities including on-call and weekend coverage.
