Job summary

Hill Physicians Medical Group is seeking a CTC Care Coordination Technician to support pre and post-acute care needs, act as a liaison among hospitals, clinicians, and care management staff, and coordinate discharge processes across Inpatient, Welcome Home, and Case Management streams.

The role emphasizes timely authorizations, discharge planning, and cross-functional collaboration to optimize patient outcomes.

Qualifications

• 3-5 years of related managed care experience
• Working knowledge of medical terminology
• Ability to coordinate effectively with a variety of customers including members, providers, hospital and office staff, health plans, internal departments, community resources, and peers
• Ability to work independently as well as in a team environment
• Multi-tasking, prioritization, and strong critical thinking skills
• Excellent organizational and communication skills and ability to meet timeframes
• Experience with CPT/ICD9/ICD10 codes preferred

Responsibilities

• Inpatient: process intake, verify eligibility, build referral into authorization, and route to clinical queue
• Welcome Home: discharge coordination, upload summaries, complete initial outreach within 36 hours
• Case Management: create episodes, outreach and refer to health plan, manager, or social worker
• Travel to hospitals/PCP offices in assigned regions as needed
• Maintain privacy and confidentiality, adhere to regulatory standards
