Qualified candidates have MSW, 3 years hospital social work preferred, LMSW/LCSW upon hire, CPR, ACM/CCM preferred, and strong communication.

Qualifications

• Master's Degree in Social Work required; LMSW or LCSW preferred.
• 3 years hospital/medical social work preferred.
• 1 year discharge planning/care management preferred.
• CPR required; ACM/CCM preferred.

Responsibilities

• Identify community resources and referrals as needed.
• Promote timely transitions to appropriate levels of care.
• Review THRIL scores and collaborate with the team on high risk patients.
• Engage in discharge planning and documentation within established timelines.
