Job summary

Texas Health Resources, Inc. seeks a Registered Nurse (RN) to support Care Transition management on a PRN basis at Texas Health Southwest Ft. Worth, 6100 Harris Parkway, Fort Worth, TX 76132. The role focuses on coordinating post-discharge care and ensuring safe hospital transitions.

You will float to different specialty areas as needed, assess post-discharge needs, and help coordinate patient care with a multidisciplinary team to ensure smooth transitions from the hospital.

Qualifications

• Current RN license in good standing
• Experience in care transitions or case management preferred
• Ability to coordinate discharge planning and post-discharge needs

Responsibilities

• Assess patients for post-discharge needs and coordinate care transitions
• Float to various specialty areas as needed to support the Care Transition Department
• Collaborate with physicians, nurses, social workers, and case managers to ensure safe transitions from hospital

Care Transition Manager RN - PRN

Work location: Texas Health Southwest Ft. Worth, 6100 Harris Parkway, Fort Worth, TX 76132

Work hours: PRN: On an as needed basis, Monday-Friday Business Hours, Between the hours of 7:30AM-5:30PM with flexibility; some weekend coverage as well. Ideally between 2 to 3 days per week.

Care Transition Department highlights:

• Care Transition management - will float to various specialty areas as needed


• Magnet status, Level II Trauma facility, Stroke & Chest pain certified, Hip & Knee Orthopedics certified.


• Assessing patients for post-discharge needs


• Helping coordinate patient care to ensure safe transitions from the hospital
