Sentara Health is seeking to hire a qualified individual to join our team as a Care Transition Coordinator.
Position Status: Full Time - Day Shift
Position Location: This is a float position that will cover locations in the Hampton Roads area where CTC is out of the office for reasons such as PTO or call out.
Responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.
Licensed RN, LPN, RT or BSW required. Proof of education as LPN, RN, RT or BSW required.
1 year of home care, discharge planning or case management experience required.
