Job Description

St Croix Hospice is hiring a Hospice Account Executive (Care Transition Coordinator) for our branch in Indianapolis. Responsible for developing and maintaining positive relationships with referral sources and communities, providing education on hospice care topics, and assisting intake to ensure timely and accurate referral coordination.

Essential Job Functions/Responsibilities
  • Meet admission and call volume targets
  • Coordinate daily sales and marketing operations, including implementing marketing initiatives
  • Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider
  • Document all sales activity in the CRM per organizational requirements
  • Employ marketing and promotional initiatives to achieve sales projections
  • Maintain comprehensive knowledge of markets, including government agencies, payer groups, referral sources, and competitors
  • Enhance communication flow between patients, families, caregivers, facilities, physicians, and vendors
  • Provide education to patients, families, and caregivers about hospice and support informed decision-making
  • Assist in establishing organization volume projections and marketing resource allocations
  • Conduct market assessments and develop marketing plans to meet volume targets
  • Coordinate marketing events, vendor fairs, and exhibits
  • Support and mentor the Care Transition Team and new staff members
  • Participate in strategic planning to identify service opportunities
  • Maintain knowledge of community resources and assist clients in accessing them
  • Monitor and report on marketing cost-effectiveness
Qualifications
  • Associates degree in Marketing, Business Administration, Healthcare, or related field
  • Ability to deal tactfully with customers and clients
  • Strong communication, negotiation, and public relations skills
  • Autonomy, organization, assertiveness, flexibility, and cooperation in responsibilities
