Qualifications

Responsibilities

• Screen patients' medical records and document relevant clinical information.
• Coordinate referrals/transfer of patients to ECFs or home care with external agencies.
• Collaborate with Social Worker/Case Manager to determine appropriate placement.
• Communicate with facilities regarding patient placement considering preferences, clinical needs and insurance.
• Investigate insurance coverage for long-term or outpatient care to facilitate transfers.
