Job summary

Synergy HomeCare is seeking a Care Team Lead to advance our home-care team in Minnesota. The role blends direct caregiving with leadership, ensuring high-quality care and caregiver/client satisfaction.

You will support training, performance feedback, and administrative tasks to keep care plans on track. The ideal candidate has at least one year of leadership experience and 3+ years as a caregiver (CNA preferred), with a valid MN driver’s license and a strong sense of professionalism.

Qualifications

• At least one year of leading and coaching others.
• 3+ years in caregiving, CNA preferred.
• Background in long-term care, assisted living, dementia care, or hospice is desirable.

Responsibilities

• Introduce new caregivers to clients, train and coach new caregivers in shadow shifts.
• Perform direct care duties as needed.
• Conduct check-ins to assess care quality and satisfaction, ensuring care standards.
• Monitor care plans and coordinate with Client Care Coordinator and Scheduling as needs evolve.
