For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, currently offering 160+ programs and services for children, youth, adults, seniors, and those struggling with mental illness.
Children’s Care Coordination provides services to children ages 5 to 18 enrolled in the various Health Homes Program. The primary goal is to provide recovery-oriented care coordination that is accessible, flexible, and tailored to individual needs. The aim is to connect clients and families with community resources to improve overall well-being and reduce emergency room visits, inpatient stays, and incarcerations.
STATEMENT OF THE JOB:
Under the supervision of the HH Team Supervisor, the Care Specialist coordinates all aspects of care for children with complex medical and psychiatric conditions, facilitating access to medical, behavioral health, substance use, social, and psychosocial services efficiently and effectively.
Responsibilities include engaging and retaining clients, developing individualized care plans, ensuring priority appointments, promoting wellness, tracking care needs, documenting interactions, and coordinating transitions and referrals. The role also involves outreach, maintaining data security, and participating in program development and evaluation.
The Care Specialist will work a schedule that includes holiday coverage, providing 24/7 availability for emergencies and support, and will report to the Behavioral Health Services Administration or Agency Administration.
Additional duties include participating in investigations, committee work, and adapting to evolving program needs.
QUALIFICATIONS:
- Bachelor’s degree in social work, psychology, or related field with two years of relevant experience, or a professional healthcare degree/certification.
- Skills in crisis intervention, time management, psychosocial rehabilitation, and linking clients to community services.
- Fluency in a second language is preferred.
