Job summary

Centerstone in Bedford, Indiana is seeking a Care Specialist to provide person-centered support to clients, helping develop life skills and personal goals. You will coordinate with a multidisciplinary team to maintain clinical records, create treatment plans, and connect clients with essential community resources.

You will also deliver crisis intervention, guide families, and ensure services align with CCBHC standards.

Qualifications

• Associate's degree required. Two years of experience or two years of college coursework in related field may be substituted for degree requirement.
• Two years of professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred.

Responsibilities

• Coordinates client care with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including treatment plans, progress notes, and correspondence with external agents.
• Teaches and trains clients in activities of daily living, individually or in groups, complying with documentation requirements.
• Connects clients with community support systems and maintains regular communications with schools and other family service providers.
• Provides telephone and on-site crisis intervention and follow-up to clients and their families.
• Understands how the job functions within the Certified Community Behavioral Health Clinics (CCBHC) model.
