Job summary

Centerstone is seeking a dedicated professional to provide person-centered assistance to clients in developing life skills and personal goals, coordinating care with a multidisciplinary team, and helping connect clients to community resources. The role includes crisis intervention as needed and documentation in line with organizational protocols.

The ideal candidate holds an Associate’s degree with two years of related experience; a full-time, on-site position in Kentucky, with Centerstone

Qualifications

  • Associate’s degree required; two years of experience or two years of college coursework in related field may be substituted for degree requirement.
  • Two years professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred.

Responsibilities

  • Coordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including treatment plans, progress notes, reporting, and correspondence with external agents, ensuring compliance standards.
  • Teaches and trains clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements, and provides services according to organizational guidelines and/or protocols.
  • Connects client with community support systems as determined to be appropriate, and maintains regular communications with schools, OFCs and other family service providers that the client and their family participates with or is receiving services from.
  • Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families.

JOB DESCRIPTION:
SUMMARY OF POSITION

Provides person-centered assistance to clients in the development of life skills and personal goals. Coordinates client care with multidisciplinary team. Assists clients in connecting to community resources. Provides crisis intervention/prevention to clients and their familes.
