Job summary

Qualifications

• 1 year of experience in care coordination, care management, or scheduling.
• Experience in home care, caregiving, or community-based healthcare.

Responsibilities

• Conduct in-home client assessments and supervisory visits to ensure safe, needs-based care.
• Develop and update individualized care plans.
• Coordinate and schedule starts of care and maintain staffing schedules.
• Build trusting relationships with clients, families, caregivers, and referral partners.
• Support caregiver onboarding, training, and ongoing supervision.
• Monitor care quality and ensure compliance with company policies and Arizona regulations.
• Maintain thorough documentation and collaborate with Recruiting, HR, and Operations.
• Identify opportunities to improve client and caregiver experience and contribute to quality improvement.
