Job summary

American Medical Administrators LTC is seeking a Care Plan Coordinator to develop and manage patient care plans with interdisciplinary teams, ensuring accurate documentation and regulatory adherence.

The role requires an unencumbered RN or LPN license and 2–5 years of long-term care experience, with knowledge of Medicare rules and a commitment to quality care. You will guide nursing staff, support utilization decisions, and maintain HIPAA-compliant records in the EMR.

Qualifications

• Unencumbered RN or LPN license required.
• 2–5 years of experience in long-term care.
• Knowledge of Medicare regulations.
• Commitment to Total Quality Management.
• Familiarity with Case Mix Index, Quality Measures tracking, QRP and Value Based Purchasing.

Responsibilities

Requirements:
