American Medical Administrators LTC is seeking a Care Plan Coordinator in Missouri. The role involves coordinating comprehensive assessments, developing individualized care plans with interdisciplinary teams, and ensuring Medicare compliance and high-quality documentation within EHR systems.

Ideal candidates hold an RN or LPN license with 2–5 years in long-term care, and demonstrate strong knowledge of Medicare regulations and Total Quality Management principles.

Qualifications

• Unencumbered RN or LPN license.
• 2-5 years of long-term care experience.
• Knowledge of Medicare regulations.
• Commitment to Total Quality Management.
• Familiar with Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing.

Responsibilities

• Conduct comprehensive assessments and evaluations of patient care needs.
• Develop and implement individualized care plans with interdisciplinary teams.
• Perform utilization reviews for appropriate resource allocation and Medicare adherence.
• Maintain accurate medical documentation and HIPAA compliance in EHR systems.
• Provide guidance to nursing staff regarding patient care and documentation practices.
