Reliant Care Management Company in Kentucky is seeking a dedicated Care Plan Coordinator responsible for assessing patient needs, developing individualized care plans with interdisciplinary teams, and ensuring compliance with guidelines and Medicare regulations.

The role requires an unencumbered RN or LPN license, 2–5 years of long-term care experience, and strong knowledge of quality measures and case-mix concepts.

Qualifications

• Unencumbered RN or LPN license.
• 2–5 years of experience in long-term care.
• Knowledge of Medicare regulations.
• Commitment to Total Quality Management.
• Working knowledge of Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing.

Responsibilities

• Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations.
• Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance.
• Provide guidance and support to nursing staff regarding patient care and documentation practices.
