Job summary

Valley Cities Behavioral Health Care is seeking a Care Navigator to work in the greater Seattle and South King areas. Candidates must have an Associate's degree and a SUDP or SUDPT credential with experience in substance use disorders.

Signing Bonus: $1500/$3000/$5000 - Match Program

  • SUDPT: $23.98 - $30.73
  • SUDP: Master's level $27.77 - $35.87
  • SUDP: Fully Licensed $31.31 - $41.01

Role Overview

Care Navigators work at various outpatient clinics in the greater Seattle and South King areas to provide MAT (medicated assisted treatment) in addition to assessment, counseling, and therapy. This position works closely with medical providers and other clinical staff to screen, enroll, and provide Care Navigation / Case Management to those struggling with an Opiate Use Disorder. The right candidate for this position would have the ability to operate in a fast‑paced environment, complete clinical documentation on time and be able to provide quality, judgment‑free client care.

Key Responsibilities
  • Assist with patient screening, program enrollment, scheduling of clinic visits, procedures and follow up.
  • Identify client needs and develop a treatment plan.
  • Provide SUD assessment and case management.
  • Partner with community service providers to facilitate client transitions.
  • Collaborate with pharmacies and Hub/Spoke team members in obtaining prescriptions, medication orders and refills, and communication as needed, in keeping with scope of practice and applicable state and federal regulations.
  • Ability to work independently and in a team environment.
  • Ability to complete documentation in a timely manner.
  • Complete data collection and enter the data into electronic health records system and tracking tools.
Minimum Qualifications

Education/Credentials

  • Associate's degree
  • SUDP or SUDPT credential are required with a minimum of 6 months of experience.
  • Ability to transport clients.
  • Current Driver’s License and Automotive Insurance
Experience
  • Experience working with clients with substance use disorder required.
  • Experience with case management or care coordination preferred.
  • Experience working with clients with mental illness preferred.
  • CPR certification preferred – may obtain upon hire.
