Job summary

SaVida Health in Bangor, ME seeks a Care Coordinator to assess, plan, implement, monitor and evaluate actions to promote patients’ physical, social, and emotional health in an outpatient setting.

Responsibilities include psychosocial assessments, strengths-based treatment planning, care coordination with outside providers, and advocacy to connect patients with community services while upholding confidentiality and professional standards. MHRTc license is required.

Qualifications

• Bachelor’s degree in counseling, social work or other allied mental health field.
• One year of experience in substance abuse treatment.
• One year of experience in mental health counseling.
• MHRTc license required.

Responsibilities

• Completing psychosocial assessments as needed.
• Conduct strengths-based treatment planning per established guidelines.
• Perform care coordination with outside providers.
• Assist with patient health promotion and wellness as requested.
• Ensure all company’s policies and procedures pertain to patient engagement.
• Develop collaborative relationships with multidisciplinary teams to support outcomes.
• Establish a network of connections with community agencies and providers.
• Provide continuity of care through documentation and communication with providers.
• Act as patient advocate to coordinate required services.
• Participate in ongoing care collaboration meetings, staff meetings and professional development.
• Participate in supervision as scheduled.
• Maintain confidentiality and safekeeping of all treatment documentation.

Education
