Qualifications

• Bachelors degree in Nursing (BSN) or Social Work (BSW) is required.
• Current, unencumbered RN or LSW license in North Dakota is required.

Responsibilities

• Coordinate comprehensive care plans and advocate for patients and families.
• Collaborate with physicians, RNs, departments and resources to identify barriers and plan discharges.
• Assess patient needs and provide evidence-based recommendations to the care team.
• Apply clinical judgment to decisions under pressure and ensure high‑quality outcomes.
• Monitor eligibility criteria and regulate resource utilization per standards such as Medicare and PRO.
