Job summary

CHI Mercy Health Valley City seeks a Case Manager to advocate for patients, coordinate comprehensive care plans, and support successful transitions for sustained well-being.

You will work with physicians, RNs, and teams to remove barriers, develop discharge plans, ensure medical necessity, and provide compassionate, evidence-based guidance.

Required: BSN or BSW and active RN or LSW license in North Dakota; prior case management experience preferred.

Qualifications

• Bachelor’s degree required in Nursing (BSN) or Social Work (BSW) or related field.
• Must hold a current licensed RN or LSW in North Dakota.

Responsibilities

• Analytical assessment to identify complex patient needs with interdisciplinary input.
• Coordinate high-risk care plans from pre-admission through post-discharge.
• Provide clinical counseling and actionable recommendations for treatment plans.
• Exercise clinical judgment to make timely decisions and ensure quality outcomes.
• Monitor regulatory criteria and optimize resource utilization.
