***Remote role. Candidate must reside in Mississippi. Preferred experience in Obstetrics / Maternal Health. Licensure is not required.***

Position Purpose

Develops, assesses, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develops or contributes to personalized care plans for members and educates members and their families/caregivers on available services and benefits to improve healthcare access and ensure high-quality care through advocacy and coordination.

  1. Evaluates member needs, barriers to care, available resources, and recommends and facilitates the best care plan outcomes.
  2. Develops or contributes to ongoing care plans and identifies providers, specialists, and community resources needed for care.
  3. Provides psychosocial and resource support to members/caregivers and care managers to access local resources such as employment, education, housing, food, and other community services.
  4. Coordinates between members, families/caregivers, and care providers to ensure timely access to services.
  5. Monitors progress towards care plan goals and collaborates with healthcare providers for revisions or further evaluation as needed.
  6. Maintains all member information and documentation to ensure compliance with relevant regulations.
  7. May conduct on-site assessments and collaborate with providers or resources.
  8. Provides education on procedures, healthcare instructions, referrals, and benefits to members and families.
  9. Performs other duties as assigned to meet business and member needs.
  10. Ensures compliance with all policies and standards.
Education and Experience

Bachelor’s degree and 2-4 years of related experience required. Clinical licensure (e.g., nursing) preferred. Equivalent experience may be considered.

License and Certification
  • Current state clinical license preferred.
  • For Mississippi Magnolia Health plan: Bachelor’s or Master’s in a healthcare-related field (social work, sociology, psychology, public health) and 2-4 years of experience; LSW or LMSW preferred.

Pay Range: $22.50 - $38.02 per hour.
