About the Role:

We are seeking a compassionate, motivated, and licensed healthcare professional to join us as a Contract Care Navigator. This role supports our Chronic Care Management (CCM) and Measurement-Based Care (MBC) programs. If you are a licensed MD, DO, NP, PA, or mental health practitioner in any U.S. state and passionate about transforming patient outcomes through whole-person care, we want to hear from you.

Key Responsibilities:

1. Deliver care navigation services via virtual platforms, including follow-ups and assessments.
2. Support patients enrolled in CCM and MBC programs using our Aspire Health Portal.
3. Coordinate care across providers, addressing physical, behavioral, and social health needs.
4. Use measurement tools to monitor progress and adapt care plans accordingly.
5. Collaborate with a multidisciplinary team to support comprehensive, whole-person care.

Qualifications:

• Active U.S. license as an MD, DO, NP, PA, or licensed mental health professional (e.g., LCSW, LPC, LMFT, Psychologist).
• Passion for integrated, person-centered care.
• Comfort with virtual care delivery and digital platforms.
• Experience in chronic care management, behavioral health, or care coordination is preferred.
