Job summary

Behavioral Health Network, Inc (BHN) is seeking a Care Navigator to support individuals involved with the criminal justice system who face behavioral health or substance use challenges in Western Massachusetts. You will help them access care, connect to resources, and build supports for greater stability and independence.

Qualifications

• High school diploma required.
• Community Health Worker certification and/or bachelor’s degree preferred.
• 1-2 years of experience in behavioral health, substance use, corrections, human services, or a related field required.
• Knowledge of mental health and substance use disorders and experience connecting individuals to community resources.
• Experience working with justice-involved individuals and/or justice system partners preferred.
• Knowledge of care management and community-based services.
• Strong communication, relationship-building, and collaboration skills.
• Spanish/English bilingual skills preferred.
• Lived experience with the criminal justice system and/or recovery from behavioral health or substance use challenges preferred.
• Comfortable using computer systems and electronic documentation.
• Valid Massachusetts driver’s license required.
• Reliable transportation required; vehicle must be properly registered and inspected in accordance with Massachusetts requirements.

Responsibilities

• Provide in-person, telephonic, and telehealth care management and support to individuals on your caseload.
• Meet with individuals in jails, hospitals, treatment facilities, and other 24-hour settings to support discharge planning and connection to community services.
• Serve as the primary point of contact for individuals on your caseload and help coordinate their care and supports.
• Complete biopsychosocial assessments, individualized support plans, safety plans, and disenrollment plans in collaboration with individuals, providers, and natural supports.
• Build collaborative relationships with probation and parole officers, court personnel, correctional staff, primary care providers, behavioral health and substance use providers, and other members of an individual’s support network.
• Help individuals access community resources, including health insurance, food and nutrition assistance, housing, financial assistance, utilities, education, employment, and legal resources.
• Maintain professional and collaborative relationships with providers, community partners, and other supports.
• Attend required meetings, trainings, and other program activities.
• Complete required documentation and other duties as assigned.

What You Will Do

Who You Are

Pay

$21.50- $25.27
