Position Purpose

Develops, assesses, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develops or contributes to the development of a personalized care plan/service plan for members and educates members and their families/caregivers on services and benefit options available to improve healthcare access and receive appropriate high-quality care through advocacy and care coordination.

Responsibilities include:

  1. Evaluating member needs, barriers to care, available resources, and recommending the best plan for outcomes.
  2. Developing or contributing to personalized care plans and identifying providers, specialists, and community resources.
  3. Providing psychosocial and resource support to members, caregivers, and care managers to access local resources such as employment, education, housing, food, and more.
  4. Coordinating between members, families, caregivers, and care providers to ensure timely access to services.
  5. Monitoring progress towards care plan goals and collaborating with healthcare providers for revisions or further evaluations.
  6. Maintaining all member information and activities to ensure compliance with regulations.
  7. Performing on-site visits to assess needs and collaborate with providers.
  8. Educating care managers and/or members on procedures, care options, and benefits.
  9. Other duties as assigned to meet member and business needs.

Education/Experience

Bachelor’s degree and 2–4 years of related experience required. Clinical licensure preferred if applicable.

Equivalent experience may be considered.

License/Certification

  • Current state’s clinical license preferred.
  • For Mississippi Magnolia Health plan, a Bachelor’s or Master’s in a healthcare-related field (social work, sociology, psychology, public health) and 2–4 years of experience; LSW, LMSW preferred.

Pay Range: $22.50 - $38.02 per hour
