Job Title: Care Navigator
Partners with Registered Nurses and other team members to coordinate care and provide support for patients enrolled in the Pediatric Complex Care Program and their families. The Care Navigator will work closely with families to ensure that they are appropriately connected to clinical, social, and community resources, and will assist families with navigation of the health care system.

Collect daily reports to identify patients and providers requiring communication regarding programmatic metrics. Prioritize and schedule communication with patients based on needs assessment and programmatic metrics. Assist in scheduling appointments with internal and/or external providers responsible for patient follow-up. Maintain ongoing contact with patient cohort regarding health care needs. Route needs to provider or ancillary staff based on urgency.
