Job summary

HopeLink Behavioral Health in Oakton, VA seeks a Care Navigator Team Lead to oversee the Central Access Team.   • Develop and maintain relationships with key community stakeholders.
  • Review monthly/annual reports for accuracy and timeliness.
  • Participate in meetings and professional development.

SUMMARY STATEMENT OF POSITION:

The Care Navigator Team Lead is part of the Central Access Team at HopeLink. The Team Lead is responsible for accepting service requests from Fairfax County residents ages 14 to 24, or their caretakers, seeking assistance to identify and be connected to mental health and/or substance use disorder services. Care Navigation provides a seamless solution that helps individuals and families access care in their communities, ensuring individuals and their families never have to navigate their needs alone

SCHEDULE & LOCATION:
  • Shifts available: Monday - Friday, 9 am to 5 pm
  • Three days a week in Oakton, VA
PRIMARY DUTIES:
  • Responds to service requests, provide an overview of Navigation Services, and schedule assessment.
  • Completes CANS assessment to determine appropriate level of care and service options. Meet with persons served and their caregiver(s), as appropriate, to review the results of the assessment and recommended services.
  • Identify appropriate service providers, taking into consideration the needs and preferences of persons served, including language. Facilitate referrals to selected providers, utilizing warm transfers when appropriate.
  • Complete ongoing follow-ups with persons served according to established follow-up cadence to ensure connection to services, and to address any issues or barriers that may arise.
  • Develop and maintain relationships with key community stakeholders with the goal of promoting Navigation Services throughout Fairfax County.
  • Review monthly and annual reports for accuracy and ensure timely submission to Fairfax County.
  • Participate in opportunities for professional growth and development to stay current on job-specific knowledge, particularly the most current resources and referrals available for clients.
  • Participate in agency and county meetings as requested.
QUALIFICATIONS:
  • Bachelor's degree and QMHP C and/or A required. Master's degree and licensed or licensed eligible preferred.
  • Knowledge of Fairfax County's continuum of mental health and substance use disorder services.
  • Bilingual – English and Spanish
  • Ability to exercise independent judgment.
  • Excellent oral and written communication skills.
  • Ability to work individually and within a team environment and adjust to changes in workflow daily.
  • Private and confidential workspace required for remote work.
EXPECTATIONS REGARDING QUALITY OF WORK:
  • Service referrals are appropriate based on assessment.
  • Provision of services are culturally and linguistically appropriate.
  • HopeLink program policies and procedures are always followed.
  • Monthly and annual reports are timely and accurate.
  • Credible documentation for the program meets program and accreditation standards.
EXPECTATIONS REGARDING QUANTITY OF WORK:
  • Respond to all service requests within one (1) business day.
  • Complete assessments within seven (7) days of service request contact, subject to the availability of persons served.
  • Review assessment results and service recommendation with persons served withing seven (7) days of assessment completion.
  • Complete all follow-ups within timeframes established in program policies and procedures.
  • Annual HopeLink compliance and training requirements are met on time.
