MindCare Solutions is seeking a dependable, tech-savvy, and compassionate Care Navigation Specialist to support psychiatric telehealth services and Behavioral Health Integration initiatives in Post-Acute Care settings. As a Care Navigation Specialist, you will work alongside a remote Psychiatrist or Psychiatric Mental Health Nurse Practitioner (PMHNP) to facilitate smooth communication and clinical workflow between the Provider, patients, and facility staff. This role is ideal for nursing students, medical assistants, or healthcare professionals seeking flexible, part-time work with exposure to psychiatric and geriatric care.
Compensation: $15-18 per hour
Schedule: M-F, 8am-5pm
