The Care Navigation Nurse LPN supports referral intake, care coordination, and initial clinical triage to ensure timely, accurate, and efficient access to hospice services. The role emphasizes one-call resolution whenever possible and seamless progression of referrals through dispatch.
This is sedentary work requiring the exertion of up to ten (10) pounds of force occasionally and some light work and lifting requiring the exertion of up to 20 pounds occasionally; work requires sitting, stooping, bending, kneeling, reaching, standing, fingering, grasping and repetitive motions; vocal communication; hearing, and visual acuity. Employee is not subject to adverse environmental conditions. This position has the potential to be exposed to environmental elements, internal and external, such as infectious disease, bloodborne pathogens, and chemical and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.
